The Transition Navigator Trial (TNT) is a new province-wide initiative evaluating the effectiveness of a patient navigator service versus usual care + newsletters to improve outcomes for patients moving from pediatric to adult care. They are looking for youth aged 16-21 years old with a chronic health condition who will be transferring to adult specialty care to participate in the study.
The navigator is a social worker who will work with youth and families for up to 24 months after the time of transfer. The navigator will support the youth and/or family during the transition to adult care following recommended clinical practice guidelines. The navigator will focus on:
*Preparing for transfer
*Reducing social and economic barriers to healthcare
*Serving as a health system broker
